Healthrite was founded by people who have spent years running healthcare services and working alongside independent physicians. We saw the same problem again and again: patients with chronic conditions do well when someone is checking in, and struggle when no one is. Busy practices, agencies and plans rarely have the staff to provide that support consistently.

Who is eligible for Chronic Care Management?
Patients with two or more chronic conditions expected to last at least 12 months, which place them at significant risk of decline, are generally eligible. Coverage depends on the patient’s insurance.
Do patients have to pay for these programs?
Programs are billed to the patient’s insurance. Depending on their plan, some patients may have a copay or coinsurance. Our team explains this during enrollment.
Does the physician stay in charge of the patient’s care?
Yes. We work under the physician’s supervision and follow agreed protocols. Clinical decisions remain with the physician.
How does Healthrite use AI?
AI helps our care team understand each patient’s needs, prepare for each conversation and spot changes early. People make every clinical decision.
Which devices do patients use for monitoring?
Depending on the patient’s conditions, devices may include a blood pressure monitor, glucose meter or scale. Our team helps each patient get set up and answers questions along the way.
How quickly can a program start?
Timing depends on the size of the patient population and the programs chosen. We agree on a launch plan together during onboarding, and our team handles outreach and enrollment so your staff doesn’t have to.
